Utility of the Addenbrooke’s Cognitive Examination in Amyotrophic Lateral Sclerosis

Objective
Amyotrophic lateral sclerosis (ALS) is a neurodegenerative condition that primarily affects motor neurons. Cognitive changes are reported in 25%-50% of patients, secondary to frontotemporal involvement. The objective of this study was to evaluate the utility of a screening tool, the Addenbrooke’s Cognitive Examination (ACE), in ALS patients.
MethodsIn this retrospective cross-sectional study, performance on the ACE was compared between 55 ALS patients and 49 healthy controls. The validation of the ACE in ALS patients was explored using a neuropsychometric battery. Correlations between the ACE and clinical variables such as the ALS Functional Rating Scale-Revised (ALSFRS-R) and forced vital capacity were computed.
ResultsA higher percentage of patients were below cut-off scores, although this remained non-significant between the patient and control groups. The ACE did not reveal significant differences between ALS patients and controls. The scores on the ACE displayed moderate correlations with our neuropsychometric battery for some domains, whereas others showed poor or no associations. Poor ACE Total was associated with lower ALSFRS-R and finger-tapping scores.
ConclusionsPerformance on the ACE was comparable between patients and controls. Associations with motor function pose a challenge to accurate interpretation of ACE performance. It is likely that patients with poor cognition have greater disability, or that poor ACE performance reflects reduced motor ability to perform the task. This raises concern for the utility of the ACE as a screening tool in ALS patients, especially since recent versions of the ACE continue to include motor-based tasks.

For her three novels, Brown Girl in the Ring (1998), Midnight Robber (2000), The Salt Roads (2003), for her collection of short fiction, Skin Folk (2001), and for her three edited anthologies, she has received wide critical recognition, including winning the Locus First Novel Award, the John W. Campbell Award for Best New Writer, the Sunburst Award for Canadian Literature of the Fantastic, and the World Fantasy Award, among others.

— Nalo Hopkinson, Midnight Robber 1Nalo Hopkinson, a Jamaican-born Canadian author, has become in less than a decade a critically acclaimed novelist of speculative/science fiction and an original voice in the critical and political debates about speculative fiction, feminism, and afro-futurism. For her three novels, Brown Girl in the Ring (1998), Midnight Robber (2000), The Salt Roads (2003), for her collection of short fiction, Skin Folk (2001), and for her three edited anthologies, she has received wide critical recognition, including winning the Locus First Novel Award, the John W. Campbell Award for Best New Writer, the Sunburst Award for Canadian Literature of the Fantastic, and the World Fantasy Award, among others. Her most recent novel, The New Moon's Arms (2007), is published by Warner/Hachett. Hopkinson attended the Clarion Science Fiction and Fantasy Writers Workshop as a student in 1995; now she is one of its teachers.
I first met Nalo Hopkinson in 1999 when she agreed to read from her new novel, Midnight Robber, at a small academic sf conference in her home city, Toronto. But that was after I had seen her in a 30-second video clip on Canada's Space Channel introducing her first novel: she was the hippest and sexiest writer I'd ever seen in my ten years of science fiction convention-going. I bought Brown Girl in the Ring and found an intelligent, inventive style and compelling characters.

Background
Bipolar disorder is a serious brain disease affecting more than a million individuals living in the USA. Epidemiological studies indicate a role for both genetic and environmental factors in the pathogenesis of this disorder.
AimTo identify RNA transcripts that are up- or down-regulated in the frontal cortex regions of individuals with bipolar disorder.
MethodSerial analysis of gene expression (SAGE) and reverse transcriptase-polymerase chain reaction were used to identify RNA transcripts which are differentially expressed in the frontal cortex of brains obtained postmortem from individuals with bipolar disorder compared with other psychiatric and control conditions.
ResultsLevels of RNA transcripts encoding the serotonin transporter protein and components of the NF-κB transcription factor complex are significantly increased in individuals with bipolar disorder compared with unaffected controls. Increased levels of expression of these RNA transcripts were also detected in the brains of some individuals with schizophrenia and unipolar depression.
ConclusionThe SAGE technique offers promise for the characterisation of complex human brain diseases.
